Job Description

Functions as a clinical expert in a specialty practice area (e.g. oncology, transplant, critical care, neonatal, and/or nutrition support). Assesses and evaluation drug therapy and makes recommendations as appropriate. Serves as a preceptor for students and residents and serves as a resource for pharmacists advancing their clinical skills. Contribute positively to the teamwork of the department and the medical/clinical team through proactive participation. Participate in planning and implementing change, while managing productivity, through attendance and participating in meetings, committees, task forces, projects and discussions with coworkers and leadership. Support assessment of staff competency and provision of timely feedback Promote documentation of interventions, adverse drug reactions and medication errors as per policy. Develop protocols, guidelines and other evidence based care models which promote the safe and cost-effective use of drugs and reduce practice variation. Develop and maintain competency with crucial steps in the medication process, including CPOE and automated dispensing cabinets, and identify opportunities for improvements. Provide direct patient care using excellent clinical skills and communications and by staying current with relevant literature. Participate in medical/clinical team rounding. Provide daily patient profile review to assess, monitor and recommend appropriate drug therapy. Identify, resolve and /or prevent medication related problems. Ensure appropriate therapy and outcomes. Works with manager to formulate plan for professional development. Attends educational in-services as appropriate. Participates in professional activities and organizations to maintain knowledge of current trends, practices, and developments. Promote an environment and culture conducive to growth in clinical knowledge, skills and application. Ensure staff understanding of clinical department practices, standards, and policies. Foster lifelong learning and development. Augment the education of pharmacists, students, medical interns and residents. Assist with the training of new pharmacy employees. Create professional development opportunities for the purpose of advancing pharmacists┬┐ skill sets as determined by the Pharmacy Practice Model.

MINIMUM QUALIFICATIONS: Graduate of accredited school of pharmacy; licensure or eligibility for licensure in Georgia; ASHP-ACCREDITED PGY1 and PGY 2 residencies or equivalent experience in the specialty area. BPS certified preferred. BLS certification within six (6) months of hire and ACLS required within one (1) year of hire, if directly applicable to practice specialty.

PHYSICAL REQUIREMENTS (Medium): 20-50 lbs; 0-33% of the work day (occasionally); 11-25 lbs, 34-66% of the workday (frequently); 01-10 lbs, 67-100% of the workday (constantly); Lifting 50 lbs max; Carrying of objects up to 25 lbs; Occasional to frequent standing & walking, Occasional sitting, Close eye work (computers, typing, reading, writing), Physical demands may vary depending on assigned work area and work tasks.

ENVIRONMENTAL FACTORS: Factors affecting environment conditions may vary depending on the assigned work area and tasks. Environmental exposures include, but are not limited to: Blood-borne pathogen exposure Bio-hazardous waste Chemicals/gases/fumes/vapors Communicable diseases Electrical shock , Floor Surfaces, Hot/Cold Temperatures, Indoor/Outdoor conditions, Latex, Lighting, Patient care/handling injuries, Radiation , Shift work, Travel may be required. Use of personal protective equipment, including respirators, environmental conditions may vary depending on assigned work area and work tasks.

The Infectious Disease Clinical Pharmacy Specialist provides dedicated pharmacy services coordinating the Antimicrobial Stewardship Program Monday through Friday. The specialist is also available to serve as a clinical resource and should be prepared to respond to various clinical questions from the medical, nursing, and pharmacy staff regarding ID related topics.
