Job Description Responsible for enhancing the patient experience by providing a seamless navigation process from diagnosis to survivorship. Responsible for collaborating with physicians and members of the interdisciplinary teams to triage, coordinate, and consistently manage patient care by educating, providing a link to research, serving as the primary point of contact for patients and families and providing indirect patient care. Works with patients and families to answer questions, provide emotional support and solve issues concerning the cycle of care.

MINIMUM QUALIFICATIONS: Graduate of an accredited school of nursing; BSN required. Two years nursing experience required, 1 year case management preferred. BLS required.

Additional Job Details

The Rheumatology Nurse Navigator will manage both pre- and post-appointment oversight of treatment of patients within Rheumatology with direct input from providers and collaboration with both internal and external ancillary staff.

Will triage patients referred to the practice for placement into the correct Subspecialty or General Rheumatology clinic, as appropriate. This will be done by working with providers to evaluate clinical and diagnostic criteria of patients spanning a wide spectrum of disease processes. Appropriateness and urgency of referrals will be assessed in order to maximize patient access.
