POSITION SUMMARY:

The Senior Care Partner is responsible for the day-to-day activities of the Partnership in Caring Program with specific duties related to the patient’s family needs and discharge planning.

ESSENTIAL JOB FUNCTIONS, DUTIES, AND RESPONSIBILITIES:

Educate our resident and/or family about normal aging and the multiple pathologies associated with the older person, the principles of rehabilitation and palliative care utilizing their nursing knowledge.

Education will begin upon admission and be ongoing throughout the patient’s stay at the facility. Serve as liaison between the resident/family and the healthcare center.

May be required to work beyond normal working hours on weekends and holidays as necessary and may be subject to callback during any emergency.

LICENSURE, CERTIFICATION, AND EDUCATION REQUIREMENTS:

Graduate of a state approved RN program from an accredited nursing school

Must hold a current RN license in state of employment

TRAINING, SKILLS, AND EXPERIENCE REQUIREMENTS:

Must have at least one (1) year previous experience in long term care
