The Evidence Based Care Coordinator provides ongoing support and expertise through assessment, planning, implementation and evaluation. The overall goal of the position is to provide successful achievement of reported metrics and to enhance the overall quality of patient care and outcomes. Responsible for the ongoing performance improvement of metrics through data review/analysis, coordination of improvement strategies, implementation of strategies that provides successful improvement of evidence based clinical approaches and care delivery. Responsible for establishing working relationships with physicians and staff through collaboration and care coordination for holistic care delivery which will produce superior health outcomes. Responsible for presentation of metrics to applicable committees and facility and division groups. Responsible for continued learning/education for all Healthcare Quality Metrics and responsible for transfer of knowledge through frequent communication with physicians, nursing, management and patient care staff. Fosters teamwork through support and accountability. The primary responsibilities for this role is Stroke Coordinator; Sepsis Coordinator and CORE measures oversight. CORE oversight includes Q-NET database management; generating reports and intense analysis of performance improvement opportunities and action plan development.

Graduate of accredited School of Nursing with Current GA state RN license, BSN required, MSN preferred. Previous clinical experience with a minimum of 3 years. Excellent verbal and written communication skills required. Posses knowledge of systems understanding, performance improvement tools/methodology and facilitation/teaching skills.
