Patient Flow RN

The Patient Flow Nurse is a professional nurse who functions under the direction of the Patient Flow Manager and is responsible for shift operations including: leading staff, activities and services, care and labor for a department within the Patient Flow division of WellStar. The Patient Flow Nurse has responsibility for implementing standards of practice, interpreting system and department policies and procedures and ensuring quality patient care and compliance with regulatory and legal requirements. The Patient Flow Nurse is responsible for patient/customer satisfaction and appropriate reporting along with coordinating department functions with the operations of other hospital departments to maximize patient flow and interdepartmental efficiency. The Patient Flow Nurse contributes to the strategic planning process by leading operations to fulfill the mission, vision, and strategic goals. He/she is responsible for employee development with an emphasis on patient flow standards.

Required Minimum Education: Graduate from an accredited RN program or equivalent required. Licensed as an RN in the State of Georgia.

Required Minimum Experience: 1 year of acute inpatient work experience preferred

Required Minimum Skills: Strong interpersonal skills, critical thinking and decision-making skills. Works well with others in a collaborative, team-focused, patient centered way. Communicates effectively both orally and in written form with all levels of internal and external resources. Analytical skills necessary to solve technical and clinical challenges. Sufficient clinical and leadership experience to ensure competence and credibility to lead others.

1. Leading maintenance of accurate and centralized information for all patients requiring placement in the hospital

a. Coordination of all procedural and surgical schedules for priority and accuracy of placement

b. Coordination of all admissions into the hospital i.e. direct admissions, emergency department and hospital to hospital transfers

c. Coordination of unit to unit transfers to facilitate opening appropriate beds as needed

d. Daily communication with bed meeting members of current hospital census and bed needs.

e. Ongoing critical thinking and use of clinical knowledge in determining the right place, for the right patient, at the right time 40

2. Leading communication of information related to patients requiring placement

a. Evaluation of all procedure and surgical schedules for patients requiring bed assignments pre and post procedure and communication of this information to the appropriate unit

b. Evaluation of all emergency department admission requests for appropriate information and determination of placement

c. Processing of information from physician’s office for accuracy regarding direct admits for proper placement and communication of this information to appropriate unit while collaborating with the Manager of Patient Flow and patient access services

d. Communication with the Manager of Patient Flow regarding all patients requiring admission from another hospital

e. Notification of Manager of Patient Flow regarding questionable admissions 40

3. Leading communication and Placement of Patients in Emergent Situations

a. Communication of patient requiring critical care placement in coordination with the emergency department, rapid response RN, and critical care departments

b. Accuracy in obtaining information to call in the On Call Teams for either Cath Lab or CVOR, calling in team, obtaining bed and notification of Manager of Patient Flow

c. Ongoing communication with leadership regarding hospital census and bed requirements in emergent situations 20

BLS is required and must be obtained by first day of employment.
