Title: DIRECTOR OF CARE COORDINATION AND PATIENT EXPERIENCELocation: ATLANTA, GAPOSITION SUMMARY:The Director of Care Coordination and Patient Experience will be responsible for implementing the newly created Perfect Path program at PruittHealth. The Director will primarily work with Nurse Navigators in the skilled nursing facilities to ensure superior care coordination while providing excellent customer service. The Director will be responsible for evaluating the Perfect Path Program (clinical care pathways) to ensure its effectiveness. The program has two main goals 1. Care Coordination and 2. Great customer service. Responsibilities include program design, program instruction, program evaluation and follow-up, collaboration, leadership, special projects and other related duties.About the Nurse Navigator ProgramThe Nurse Navigator is responsible for the programs and resources that support the navigation of patients through complex healthcare environments for the optimization of care and outcomes. The Nurse Navigator facilitates patient follow-up through coaching, information transfer, and communication in order to improve continuity of care, enhance the medical management of complex disease, and streamline the intervention process. The incumbent guides patients through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes. Serves as a point of contact for referring physicians, patients and caregivers to provide resources and assistance with accessing clinical and supportive care services.ESSENTIAL JOB FUNCTIONS, DUTIES, AND RESPONSIBILITIES: Proven track record in project management, care coordination, and customer service. Demonstrated ability to lead a multidisciplinary clinical team to achieve key clinical and operational metrics. Demonstrates ability to collect, compile, analyze, and utilize data.Prepares written monthly reports for the Care Integration Officer to include accomplishments, goals, and recommendations for improvement. Demonstrated ability to apply comprehensive knowledge of the fundamental of the nursing process, standards of care, and demonstrates sound decision making and judgment in educational activities involving all levels of patient care personnel. Demonstrates ability to develop and evaluate educational programs for a variety of adult audiences. Demonstrates the ability to articulate clearly and make presentations before small and large groups across all age cohorts. Prioritizes the development of educational programs according to identified learning needs and objectives.Analyzes and utilizes evaluation feedback to assist in the revision and development of current and/or future workshops and conferences. Prepares written monthly reports for the Executive Director of Clinical Development to include accomplishments, goals, and recommendations for improvement. Implements employee relations practices necessary to establish a positive employer-employee relationship and promote a high level of employee morale. Maintains clinical and education competence through continuing education, seminars, workshops, journals and professional organizations/certifications.LICENSURE, CERTIFICATION, EXPERIENCE AND EDUCATION REQUIREMENTS: Bachelor of Science in Nursing (BSN) preferred. MBA in Health or related field Certification in Cardiopulmonary Resuscitation. Minimum of five years of professional healthcare field.
