Primary Responsibilities: Coordinate inbound / outbound calls - patient calls, appointments, DME, referral updatesSchedule, track and confirm various types of Nurse Practitioner (NP) home visitsDocument outreach, NP visit activity and case information completely, accurately, and in a timely manner in EHRMaintain and update team risk lists Provide daily support to NP’s in the field as needed (confirm addresses, phone numbers, reschedules, cancellations)Identify and update demographics, member contact info and care levels in EHRFax NP encounters to PCPsFax patient orders to vendors (i.e.

lab companies)Notify pharmacies of new prescriptions and authorizations of refills as ordered and / or authorized by the ProviderParticipate in quality monitoring process and closing gaps in care as directedDemonstrate a high level of customer service behavior at all timesExpedite emergency calls as neededAttend regular staff meetings as scheduledMaintain confidential health information according to state and federal regulations including HIPAAAssist with gathering and organizing of appropriate medical records (e.g.

history and physical, lab work, test results)Performs other administrative and care coordination duties as assigned by supervisorCoordinates appropriate referrals to case management staff Demonstrates sensitivity to issues and show proactive behavior in addressing customer needsConducts urgency screens of new members and triages as neededMonitors EMR for note completion and assigns patients to appropriate provider and programDevelops program reports for internal and external customerPrepare materials for meetingsCoordinate supplies for providers in the field, ordering, distributing etc.Gather and organize information for leadership to distribute to teamPerform data collection, data entry and quality monitoring activities, as applicable Required Qualifications: High School Diploma / GED2+ years of telephonic Customer Service experiencePrior Healthcare experiencePrior experience with Medical TerminologyExperience working with Microsoft Word and Microsoft ExcelPrevious experience utilizing a PC Preferred Qualifications: Associate’s Degree (or higher)Experience working with chronically ill populationSoft Skills: Strong Organizational skillsStrong Verbal and written Communication skillsCareers with Optum.
